Hello.
I've encountered a couple of issues when attempting to specify an alternative to uber-trace-id as the HTTP header used for forwarding opentracing information:

  1. APIcast/docs/parameter.md . Minor problem: Should be OPENTRACING_FORWARD_HEADER (not OPENTRACING_HEADER_FORWARD)

  2. When adding an env var of OPENTRACING_FORWARD_HEADER set to: x-request-id, apicast bombs with this stack trace

I'm currently using the following version of apicast:

registry.access.redhat.com/3scale-amp24/apicast-gateway:1.13
