Default: More in-your-face, Handout: more beautiful

Currently the "default" and "handout" modes are similar.

However, I think the default needs to be more in-your-face --- for one thing, let's box environments so that the nesting is completely clear to a potential author.

Then, let's make the default "handout" look nicer.
