Could you share the nginx config used at insight.bitpay.com? I can't figure out how to make my insight instance run on a subdomain without a trailing /insight/

It's actually not an nginx configuration option, you can change bitcore-node.json to adjust the URLs. Here is more information: bitpay/insight-api#430
