In ACF (2016 and 2018), if addRows() is called with the includeQueryColumnNames option, the columns row is added in alphabetical order and in upper case instead of the original order and case.
The bug also applies to workbookFromQuery() with the addHeaderRow option, and also affects reading spreadsheet files into CSV and HTML.
