  1. (A) ACT Research/ Comorbidities/Charlson comorbidity: After h_level 5(at the level when ICD9, ICD10 codes are added), HT terms and PT terms, they all appear as Leaf nodes. Hierarchical pattern is missing.
    Examples:
    • Under ACT Research/Comorbidities/Charlson Comorbidity/Charlson Comorbidity/AIDS HIV – HTs like ‘042-042.99 Human Immunodeficiency Virus [Hiv] Infection’ appear along with PTs like ‘042 Human Immunodeficiency Virus [Hiv] Disease’.
    • Under ACT Research/Comorbidities/Charlson Comorbidity/Charlson Comorbidity/Malignant Cancer: both HT terms like ‘140-239.99 Neoplasms’, ‘140-149.99 Malignant Neoplasm Of Lip, Oral Cavity, And Pharynx’, ‘140 Malignant Neoplasm Of Lip’ appear as Leaf nodes along with real Leaf nodes like ‘141.2 Malignant Neoplasm Of Tip And Lateral Border Of Tongue’.
    2 (B) ACT Research/ Comorbidities/Elixhauser comorbidity:
    (i)Tool tip for all the terms from h_level 3 – to be changed; appearing as “undefined” followed by ICD10CM code.
    (ii) No children under Cerebrovascular disease (POA)
    2 (C) ACT Research/ NIH Enrollment – all terms are appearing like HT but without any children
    2 (D) ACT Research/Phenotyping – CCS,HPO, LOINC2HPO, Phecodes coming under this?
    2 (E) Scores has Loyalty
    Only high – level terms are present without the tree.
