Incomplete signed document being sent to all parties

In a strict 3-person signing workflow, after the 2nd person signs the document, all parties received the incomplete signed document emaild attachement with the 3rd person missing signature. The email notification says "All invitees have signed the attached document."

Steps to reproduce:

1. Invite 3 users to sign a template with a strict 3-step signing workflow.
2. The 1st user signs.
3. The 2nd users receives the email notification to sign.
4. The 2nd person signs.
5. The software skips the 3rd person by not notifying them of their turn to sign.
6. All 3 users received a copy of the incomplete unsigned document (missing the 3rd user's workflow and signature).

@rubyonrailsstarter unfortunately we were not able to reproduce the issue - could you please share 3 links to sign some sample document with a reproducible issue?
