ShUp: configurable study cards (import MR)

I hope you are doing fine.
The goal of this issue is the same as in issue #2112, but I created a new issue for ShanoirUploader development.
We should extend ShanoirUploader, depending on the specific study configuration, to act accordingly:
ask for a study card selection, if mandatory, do not display selection, but propose center + equipment selection
in ShanoirUploader, according to the webGUI, see pull request #2456.
The new implementation should not impact already installed "old" ShanoirUploaders.
The import on the server will have to be tested for MR, if no study card ID is sent, that objects are created correctly on the server.
