Add a code of conduct

My Pain

I want the Panel and HoloViz community to be a great place for users, contributors and other interested to meet. I believe something like a code of conduct is a great signal of inclusion and caring. And it would also come in handy the day we experienced some conduct issues.

This request is for the github repository. I would propose we reuse an existing instead of inventing from scratch.

I can see our community discourse faq includes a code of conduct but does not call it that. I would propose including the term code of conduct so that it searchable etc. See https://discourse.holoviz.org/faq
