Update 21 US (approved by MON) Registration of the service provider

As a user (service provider) I want to be able to register in the application so that I could have further access to all the application’s features and member privileges.

Preconditions:
• A user doesn’t have an account in this application yet.
• Registration selection is described in the story “Registration choice #37”
Acceptance Criteria
1. After choosing the button «Зареєструватися як заклад позашкільної освіти» new page appears with three buttons:

2. After choosing the form of ownership, new page appears with text message “Будь ласка, внесіть дані:”
   This page contains further information for filling:

3. After pressing the button “Зареєструватися” the pop-up window appears which says: “Листа з подальшими інструкціями було відіслано на Вашу пошту, вказану при реєстрації. The letter is sent to the mail, where the customer needs to confirm the registration.
4.After pressing the button “Відмінити”, all entered data is erased and you return to the main page.
5. If any point in obligatory information is not provided, including checkbox checking, the button "REGISTER" should be unavailable.
Out of scope: 

1. Sending the registration data of the company to the administrator from MON for verification. The result should be an e-mail to the company with approval/refusal/notes of registration (with the reason if refusal).
