Add templates for Issues and PRs

Issues and PRs are easier to handle when they have templates.

Between the two langchain repos, there are now thousands of issues open with no templates. It is incredibly challenging to navigate and understand the totality of the development landscape when no templates are available.

Only a maintainer or repo owner can implement this.
