Commit 6125cf9 is available at https://github.com/nanch/phpfmt_stable and is the current reference for the sublime text phpfmt package on packagecontrol: https://packagecontrol.io/packages/phpfmt
Installing from packagecontrol is the recommended method.

How to revert to an older version of phpfmt manually:
- To revert to build 6125cf9 available at https://github.com/nanch/sublime-phpfmt/tree/6125cf9058c0666f06ed758f0f5451996f7c7211.
- Clone commit 6125cf9 into C:\Users\Me\AppData\Roaming\Sublime Text 3\Packages\phpfmt and everything will work fine again.
