The PDF/A document has an action specific to forms.

Reproducibility

Adobe Acrobat X Professional

If a form button does not exist, enter form edit mode

Tools: Forms: Edit,

then

Add New Field list: button.

When a form button is available,

open the properties for the button,
switch to Actions tab,
select trigger, for example "on Mouse Up",

then select action...

Reset Form

... Reset A Form,
and either select which fields to reset or select all.

Import Data

... Import Form Data,
and Select a FDF file.

Submit

... Submit A Form,
and either send the form to a web server by URL or to an email address.
