Extension catalog authors should be able to define default extensions for certain labels, given that multiple extensions could be associated with the same label(s).

is that done today somewhere in the existing setup ?

really should rename label to keyword since then this makes much more sense.

is this just about having i.e. camel able to put "camel", "integration" in all their extensions keywords ?

if yes, I suggest we do that as having extension mojo read quarkus.extension.metadata.keywords property and add them in if nothing in the .yaml.

Then they can define that property and use maven notion of sharing it where it makes sense.

ah - I think you mean that when using addextension using a label the platform/catalog could declare which extensions should "win" ?
