Notification Number Doesn't show #4

When a notification comes up it updates the Page header to say (#) but on the page itself the number doesn't show over the notifications icon. If you refresh the number will show up on the icon but the moment it refreshes to check for notifications it disappears again.

This has been the case since install but I'm currently running 4.0.9.2

Now running 4.0.10.2 and still the same.

I'm getting this exact error on 4.0.10.2.
