When you disconnect from the server whilst in a map instance, it doesn't delete them.
When you rejoin, you spawn again in that map and not in the main world.
You then cannot leave the map, since it is not registered as an active adventure anymore.
Maybe you can add a check if after 5min there are still people in a map, and if not: delete it.

I can't replicate it on my test server. What is the context in which this happens? Team size? Number of teams?
Posting a log will be helpful too. The recommended build does not contain a log file but the version found in branch unix-zip-fix does. You can build it from there or get it here https://dl.dropboxusercontent.com/u/19977649/AdventurePlugin/AdventurePlugin-2.1.1-Beta.jar. Run that and it will generate a log file
