Update references made to UCR URLs

#152 changed the referencable URLs of the UCR document, however did not include updates to existing references.

Documents that need to be updated:

• https://github.com/solid/authorization-panel/blob/master/proposals/authorization-ucr/uc-survey.md (may also require content re-organisation and having survey responders to update/verify)
• relatively minor: references made from this repo and probably elsewhere

AFAIK - the references in this document were all updated in #152. Can you point to broken links - or just fix them?

I neither broke the references or approved the PR, so I suggest others are assigned to this issue.
