Unresolved horizontal review issues

Thanks to @himorin for opening w3c/ttml2#1228 and w3c/ttml2#1229 as discussed last meeting.
We still need to address w3c/ttml2#1211 via its break-out issues w3c/ttml2#1212 and w3c/ttml2#1213 - @skynavga offered to review those and propose a change.

With reference to w3c/ttml2#1189 (comment) is there any further change that we should be making?

Shear calculations and origin of coordinate system. w3c/ttml2#1199

See also google/ExoPlayer#8720 (comment) - there is implementer speculation about the need to apply a scaling factor as well as (or as part of ) a shear transformation in order that the resulting skewed box does not exceed the bounds of the surrounding box.
