This PR adds GitHub issue forms with the goal of guiding users in creating useful & actionable issues.
I've got this merged on my fork, so you can play around with the forms and/or create test issues there. I'll try and keep my fork's main branch in sync with this PR for testing purposes.
I'm open to a lot of feedback on this; it's definitely not easy to strike the balance between cultivating a useful issue and overwhelming end-users with too many questions.
Alternatively, we can merge this more-or-less as-is and then iterate over time.
