🚀 Major feature request: Implement MCHAT-R/F and MDAT questionnaires

Use Case

Health care providers need the respective questionnaires to assess the patients conditions. We focus on two different questionnaires: MCHAT-R/F and MDAT.

The contents of the questionnaires should be implemented accordingly considering different language options for the patients/health-care providers.

Additional context

One might need to elicit more requirements on the topic on how to integrate the questionnaires into the app or how to integrate them best into the Schedule tab.
