Create Item Template for Page/PageModel

We will need an item template for Page/PageModel, and/or a dotnet new item that does the same.

our project template work will go https://github.com/dotnet/templating/tree/rel/vs2017/post-rtw/template_feed/Microsoft.DotNet.Web.ProjectTemplates.2.0

We will get an update in a few days about whether additional work is needed to bring this into VS.
