Add a counter field to the Submission model/table/DAO/DTO.

The counter field should increment for each submission for a given LEI + period (filing id)

Use the counter field in place of the submission id for report names and sign and submit confirmation_ids.

Put a constraint so that filing id + counter is unique for the submission table.
