There seems to be a race between installing initial breakpoints and launching/attaching.

After some debugging, I see that node.js debug adapter sometimes cannot find source maps for this configuration.

Or maybe we should await initialized event: https://microsoft.github.io/debug-adapter-protocol/specification#Events_Initialized
