Toss out almost all references to 'stream_positions'

_current_positions will have no values at this point in startup, as it's near the beginning of the init process. Just set it to the 1 as the default.

Replace the SQL loading positions from stream_positions at start with new SQL looking up the data directly in the associated tables for a given stream writer
