Installation

  1. Download SimplePhpFormBuilder.
  2. Add SimplePhpFormBuilder folder at the root of your web server (http://localhost).
  3. Enter to your MySql database server and create database called 'formbuilder'.
  4. Enter to application link (http://localhost/SimplePhpFormBuilder).
  5. Start installation process:
    step 1: If everything goes well, the installation process starts and you see the next page: Choose "Follow the wizard to install your program" and click "Start".

step 2: the system checks the server requirements. If all the requirements passed, click "Continue".If not, handle the problem accordingly.

step 3: Database Settings:

Database Host Hostname or IP-address of the database server.
Database Name The database used to hold the data : 'formbuilder' (Can not be changed).
Database Username The username used to connect to the database server.
Database Password The password is used together with the username to connect to the database server.
Installation Type Choose "New".
Click "Test Connection" if everything is OK Click "Continue", if not, check the data entered and try again.

step 4: Administrator Account: Click "Continue".

step 5: Ready to Install: Click "Continue".

step 6: Installation completion - If everything is completed successfully you will see the next page: Click the "Proceed to login page" link or insert the link on the homepage such as: http://localhost/SimplePhpFormBuilder/

step 7: Login page: Click "Login". Enter user name : "Admin"
password: "Admin123".

step 7: Main application page: After entering the main page, we recommend changing the password of Admin user.
