Avoid closing the underlying XMLStreamWriter too early
e9b20e6 
Contrary to other implementations, Saxon closes the passed
`XMLStreamWriter` after transforming DOM to StAX which caused a
`NullPointerException` on the subsequent call to `flush()`.

Fixes #315.
