Use the terminology from the specifications ("status responses"), but
include a note that these are different from "STATUS" responses.

The planned `:future` config value is `:frozen_dup`, not `:raise`.
This styles attr accessor headings more similarly to method headers. Additionally, since a background color has been applied, the link underlines seemed to be too much. So this disables attr/method header link underlines too.

These are some relatively minor rdoc updates, mostly for response data, and mostly just to make the documentation slightly more consistent.
