According to the spec, if minimumUpdatePeriod is not present, the MPD should not be updated.

This will require refactors to SegmentIndex to generate references without fetching a new MPD and processing it.

Choose live playback by type='dynamic'.

Also avoid fetching manifest updates when minimumUpdatePeriod is
missing.  In the absence of minimumUpdatePeriod, just reprocess
the MPD we already downloaded.

Closes #69, #70.
