Sync up with latest version of qualifier repo

I updated the docs in qualifier, and some of those changes should be represented back in this repo.

This is broken into separate commits for easier reviewing.

• More date fields
• Add DiagnosticReport Lab profile (and tweak DiagnosticReport Note profile to only look at non-Labs)
• Rename c_term_coverage -> c_system_use
• Rename q_term_use -> q_system_use
