Add center director quarterly report template

Description

Adds a new class for generic report templates and a center director report that uses that class.

Motivation and Context

We need report templates.

Tests performed

Added integration test to check that access is limited to the expected user types. Manually tested creation of reports from report template.

Types of changes

• Bug fix (non-breaking change which fixes an issue)
• New feature (non-breaking change which adds functionality)
• Breaking change (fix or feature that would cause existing functionality to change)
